The Kinetix Group’s Care Delivery Consulting Lead, Anna Thomas, Selected to Speak at PCMH and Practice Transformation Summit

The Kinetix Group (TKG), a leading life sciences marketing agency and care delivery consultancy, will participate in the World Congress PCMH and Practice Transformation Summit on May 22-May 23 at the Wyndham Orlando Resort International Drive. Anna Thomas, Vice President of TKG and Assistant Director of the Practice Transformation Network, will share her expertise in health system transformation with a specific focus on patient-centric care management.

The PCMH and Practice Transformation Summit, now in its third year, explores best practices in primary care transformation and quality improvement through the lens of the Patient Centered Medical Home model. The conference is an opportunity for leaders of health systems, PCMHs, ACOs, primary care practices, FQHCs, health homes, and health plans to share actionable solutions in improving and accelerating team-based care.

During the event’s “Optimizing Care Management and Care Coordination Effectiveness” panel, Anna will share best practices and lessons learned through the firm’s work with leading health systems and physician groups across the country. She will also discuss the challenges she and her colleagues at the QualityImpact PTN see facing their practice constituents – now over 120 strong – as they seek to improve quality, reduce cost, and improve care for their communities.

TKG’s care delivery division has extensive experience partnering with some of the nation’s most prestigious health systems to redesign and optimize care management models. Specifically, the firm assists clients with:

  1. Establishing best practices and innovative models for care management
  2. Identifying patients for care management and care planning
  3. Improving collaboration between care managers, providers, and patients/caregivers
  4. Designing effective and efficient care management models congruent with practice resources, structure, and culture
  5. Implementing workflows to enhance the standardization and systematization of care management interventions
  6. Aligning program design to reimbursable opportunities
  7. Developing measurement and reporting strategies to demonstrate program impact/RO

PCMH topics that will be discussed during other panels include practice integration models, staff training tactics, the role of payers in practice integration, and improvement of SUD management in primary care, to name a few.
